Actor Contract

I, ………………………………………., give my permission to work for Bethan Parry, Theo Daniels and George Whiting in the making of their music video. I allow this team the rights to film myself in their music video and later on edit any clips I feature in.

I agree to give this team the rights to use the clips I feature in, anywhere they wish. I also agree for the clips I feature in to be shown to any audience in any form.

I agree that I cannot argue with this team’s rules or choices after signing this form.
